Most Solutions Actually Make Things Worse
Healthcare organizations typically respond to cognitive overload with more checklists, more protocols, more systematic processes. We’re doubling down on the very thing that created tunnel vision.
These “solutions” are cognitive load multipliers disguised as helpers. We add documentation requirements thinking we improve safety, but fragment attention further. We create alerts and notifications thinking they’ll catch problems, but add to the noise providers must filter through.

Measuring Success By What Disappears
We’ve learned to flip our measurement approach entirely. Instead of tracking positive outcomes, we measure negative events that disappear.
When we redesigned medication ordering to eliminate unnecessary decision points, we didn’t track “physician satisfaction scores.” We tracked the 40% reduction in pharmacy callback interruptions and 23% decrease in medication errors from incomplete instructions.

When we discover shadow workflows, our opening conversation is always “thank you for showing me where our design is broken” rather than “you shouldn’t be doing this.”
We formalized “workaround rounds” where staff explicitly show us their shortcuts and system hacks. We frame it as system improvement research, not compliance auditing. Anything shared gets used only for redesign, never disciplinary action.
The breakthrough comes when providers realize we’re genuinely eliminating the problems that created their workarounds. But you must fix underlying issues quickly, or people lose trust in the process.
We create environments where people feel safe saying “I’ve been doing this dangerous thing because your system made me choose between safety and sanity.” Then we actually address that impossible choice.
AI as Cognitive Infrastructure, Not Cognitive Assistant
Our biggest concern with AI implementation is creating “AI cognitive debt” where we automate easy parts but leave humans managing exceptions and edge cases. That’s the most mentally taxing work.
Some AI documentation tools handle routine notes beautifully, but when AI gets confused, providers become detectives figuring out what the system was thinking, correcting mistakes, documenting overrides. That’s more cognitively demanding than writing notes themselves.
Organizations that succeed treat AI as cognitive infrastructure, not cognitive assistance. They’re not building AI to help doctors make better decisions. They’re building AI to eliminate entire categories of decisions that shouldn’t require human judgment.
Stanford’s ambient AI pilot showed 96% of physicians found the technology easy to use, with 78% reporting it expedited clinical note-taking.
What works is “cognitive segregation.” Clearly define what AI owns completely versus what humans own completely, with minimal overlap. AI handles routine pattern matching, data aggregation, and administrative workflows invisibly. Humans focus exclusively on complex clinical judgment, patient interaction, and ethical decisions.
From System Survivors to System Architects
The most surprising resistance to invisible AI solutions came from experienced providers concerned about losing professional identity. Some of our best clinicians had built self-worth around navigating complex, broken systems.
They took pride in remembering workarounds, knowing which buttons to click in what order, being the person others consulted when systems were difficult. When we made processes invisible, we accidentally threatened their expertise.
We learned you can’t remove cognitive load without replacing it with something meaningful. We reframe expertise from “knowing how to work around problems” to “knowing how to solve problems that matter.”
Veteran providers become cognitive load detectives. They spot unnecessary complexity because they’ve lived with it longest. That charge nurse who was the go-to person for system workarounds? She became our lead on redesigning handoff protocols because she understood how cognitive fragmentation affected patient safety.
Instead of being the person who knew which buttons to click, she became the person who eliminated the need for those buttons entirely.
The Fundamental Mindset Shift
Healthcare organizations must stop asking “how do we make people better at managing cognitive overload” and start asking “what cognitive work should humans never have to do in the first place?”
We can’t solve cognitive overload by teaching people to be better at managing cognitive overload. That’s like trying to solve traffic by teaching people to be better drivers. You have to change the fundamental design of the system.
The most effective interventions are invisible to end users because they eliminate problems before they register as problems. When we get it right, the feedback is “I don’t know what changed, but work just feels easier lately.”

We measure success by what people don’t notice rather than what they do. The goal is removing cognitive friction, not adding cognitive support. It’s the difference between giving someone a better map versus removing obstacles from their path entirely.
